Patients with health care insurance:
We are in contract with a number of insurance carriers. We currently accept the following insurances:
- Medicare
- Anthem Blue Cross Blue Shield – Commercial HMO and POS
- Humana
- Network Health Plan – Commercial HMO POS, Marketplace, Medicare
- Trilogy Health – Commercial
- United Health Care – Commercial
- United Health Care Medicare and Retirement – Medicare
- Exceedent LLC – Commercial self-insured
- Sheboygan’s Employers Health Network – Commercial self-insured
For physician-ordered lab diagnostics or imaging studies (X-ray, ultrasound etc…), the insurance carrier will pay the fees at the same rate as if they were ordered from a traditional office. Patient may be responsible for the copays according to your insurance carrier requirements.
Travel Fee
Travel fee compensates for our travel time associated with seeing patients at your home or office instead of the physician’s office. This fee is not covered by insurance and is due upon the arrival of the provider and is not refundable. Travel fee is charged through your credit card. Travel fee applies when seeing individual patient in private residence, office or a hotel. Travel fee is waived for multiple patients on the scheduled dates, like when seeing patients in an assisted living facility or other communal settings.
The standard travel fee is $99 within 25 miles from our Oak Creek Office (7001 S Howell Avenue, Oak Creek, WI 53154) as calculated based on Google maps. For travels between 25-40 miles the travel fee is $149. For travel distances more than 40 miles please call for more details. Patients without Insurance – Incomplete
Patients with no health care insurance:
For patients with no insurance or who would like to not utilize their insurance, we charge a flat fee of $199 per hour with no application of travel fee. One hour would be the minimum visit charge, which would be done through a credit card. If patients need lab work or imaging, they are billed additionally for those services or they may utilize their health insurance for coverage
